Nomination Forms
Nominations of people who are mainly involved in particular sectors for example education, transport, the arts and local government can send the forms directly to the government department responsible for the sector. A nomination for a teacher who also does charity work can be sent straight to the Department for Education and Skills and someone who is involved with sport at grassroots level can be sent straight to Department for Culture, Media and Sport.
Below is a list of departments that cover particular sectors.
Department for Education and Skills
- Generally anyone involved with a school, college or university for example headteachers, teachers, school assistants, mid-day supervisors, school secretaries, school governors, university vice- chancellors.
- Anyone involved in youth work such as youth club, boy brigade, brownies, cubs, girl guides and scouts, foster carers, children's charities, play and nursery schools.
- Those involved with lifelong learning or skills training: for example tutors in adult education centres, people working in further education colleges and in the learning and skills council.

- All council members and some staff excluding specialists who are served by another Government Department, such as social workers (Department of Health) or tourism managers (Department of Culture, Media & Sport).
- Cartographers and related specialists
- Community workers and volunteers where they are, for example, involved with either a New Deal for Communities project or a particular housing estate, including tenants' / residents' association activists.
- Firefighters and their support staff.
- Homelessness activists, both employed and voluntary
- Housing association staff and board members
- Planners and developers (both private and public sector, but excluding architects, surveyors and those directly involved in the construction process).
- Regeneration specialists (both private and public sector).
- People dealing with equality issues which includes race, faith, gender and sexual orientation.

- Employees, officials or specialists working in the aviation industry including airlines, airports, incident investigations, air security and air traffic control
- Employees and specialists in road construction and road maintenance
- People at all levels including volunteers dealing with road safety
- People at all levels working in or dealing with local transport including local roads, buses, trams, taxis, cycling and pedestrians
- People at all levels involved with shipping, seafaring, ports and maritime search and rescue
- People working in over or underground railway systems
- Workers and volunteers dealing with mobility and access for people with disabilities.
